I tried to match the above to my book "Kurs for utdannelse av soppsakkyndige" to the best of my ability:
HATTENS FORM
Campanulate = ?
Conical = kjegleformet
Convex = halvkule
Depressed = nedsenket midte
Flat = flat
Infundibuliform = traktformet
Ovate = klokke (?)
Umbilicate =navlet (?)
Umbonate = puklet
WHAT ABOUT: "kule, hvelvet, traktformet med papille and spisspuklet" in English?

SKIVESTILLINGENE
  Free =Fri fra stilk (fluesopper)
Adnexed=Avrundete skiver
Adnate =Tilvokste skiver (kragesopp)
Decurrent = Nedløpende skiver (gulmosehatt)
 Emarginate =Utrandete skiver (slørsopper)
Utrandet med tann (musseronger)
Helt atskilt fra stilken (store parasollsopper)

UNSURE REGARDING: "seceding, sinuate and subdecurrent" in Norwegian, and
"utrandet med tann, and helt atskilt fra stilken" in English.
